At Little Tigers, every day is built around play — because that's how young children make sense of the world. Our teachers follow each child's curiosity, turning a puddle, a picture book, or a pile of blocks into a chance to learn something new.

We keep our groups small and our doors warm. You'll always know how your cub's day is going, and they'll always have a grown-up nearby who knows them by name.

Part A

California state-required forms

Title 22 parent forms from the Dept. of Social Services (CDSS). Download each official form, fill it out, then upload the completed copy here so it travels with your packet.

Two forms your child's doctor completes

State law also requires a Physician's Report (LIC 701) and an Immunization Record. These are filled out by your child's health provider, so they can't be signed here.

Physician's Report (LIC 701): download it below, bring it to your child's appointment, and return it before the first day.

Immunization Record: you don't need to download anything. Your child's doctor or clinic will give you the immunization record (often called the yellow or blue card) at their visit. Just send us a photo or scan of the copy your provider gives you — you can upload it with the state forms above, text it to us, or bring it in before the first day.
